Long term care waiver services are particularly developed and designed for people who are physically and mentally disabled. Majority of these clients require a level of care at a nursing facility, nonetheless, prefer to live in their own home as independently as possible. TLC in Action Home Healthcare supports our clients’ wish to have the option to receive the same level of care in their own home, instead of a facility. We support Consumer Directed services, which enables the client to hire a family member, or friend or anyone to perform them personal care services. We also provide Agency Directed services, in which we will work with our clients to develop their plan of care, and find the competent, skilful care provider to meet those needs.

    Our consultants are highly knowledgeable and have the up-to-date information about the State, local and Federal home care regulations. We offer consultant services for new and existing home healthcare agencies by offering the following services:

    • Initial Licensure Application Process
    • Development of Home Healthcare Agency Policies & Procedures
    • Education of Regulatory requirements and Home Healthcare Regulations (State, Local & Federal)
    • Annual Review of Policies and Procedures
    • Support with Quality Improvement (Problem Identification, Root Cause Analysis, Action Plan, Implementation Plan and Ongoing Monitoring), using the Lean Six Sigma Methodology for process improvement and problem solving.
    • Support with Corrective Action Plans to meet the regulatory requirements of the Local and State Governments.
    • Support with Initial and periodical surveys.

    We are a VA Medicaid licensed and certified agency to offer a comprehensive Personnel Care Aide (PCA) training. The training is required by State of Virginia Medicaid for all the providers caring for the clients with VA Medicaid. This 40 hour long training follows the curriculum approved by VA Medicaid, and exclusively covers all components of personal care activities. At the end of successful training, the trainee will be deemed ‘Certified Personal Care Aide’.
